How much do your services cost?

Most insurance plans provide coverage. Fees for self-pay clients are based on a sliding fee scale, which takes into account family income and the number of dependents. We have a variety of services, and the cost varies depending on what services you need. In order to determine your service needs, and the associated fees, please schedule a screening/intake appointment by calling (904) 209-6001.

What hours are you open?

SJCMHD is open for screening appointments from 8 am to 4pm, Monday through Friday (except county holidays). All other outpatient therapy and psychiatric services are by appointment, with sessions beginning as early as 8 am to as late as 6 p.m., Monday through Friday (except county holidays). In addition, emergency/crisis services are available through Flagler Hospital's Psychiatric Emergency Services 24 hours a day 7 days a week by calling (904) 824-HELP (824- 4357). Crisis services may also be obtained at SJCMHD during regular business hours (8am - 5pm, Monday through Friday) by calling (904) 209-6001. Are your services confidential?

Yes. Your identity and the services you receive will not be revealed to anyone outside the agency without your consent. (There are some legal limitations to confidentiality, for example - abuse of children. The parameters of confidentiality are reviewed during the screening/intake appointment. How can I make a referral?

Referrals can be made by the courts, by the client themselves, by the client's family, or by another agency directly working with the client. To make a referral, simply call (904) 209-6001, Monday through Friday between 8 am and 5pm. Referrals made to SJCMHD are not a guarantee of treatment by the agency. Rather, each referral is carefully evaluated by staff to determine whether the agency can provide the appropriate services. Regardless of the source of the initial referral, it is the client or client's guardian that must establish the appointment for a screening/intake session by calling (904) 209-6001.

Where are you located?

SJCMHD is located at 1955 U.S. 1 South, Ste. C2, in St. Augustine (inside the Flagler West building, with the entrance on the east side facing U.S. 1 South).
