THERAPIST IV - MENTAL HEALTH
St. Johns County's Mental Health Department seeks a Therapist IV to provide group/individual counseling and related clinical services to child or adult clients and their families in accordance with Medicaid documentation requirements, continuity of care and Baker Act Standards, and Center Policies and Procedures. Main Responsibilities are to complete evaluations/intakes, assessments, and formulate treatment plans/treatment reviews for assigned clients. Provide individual/group/family therapy, education, prevention and intervention services. Services will be provided to clients both on-site and off-site (e.g., clients home, school, etc.), as needed to facilitate change. Assure adequate and complete documentation of treatment plans, progress notes, disposition, referral data, instructions, termination reports, data reporting and all other required department documentation for assigned clients. Maintain collaborative efforts and coordinates needed ancillary services with other staff, physicians, agencies, and facilities in best interest of assigned clients. Attend various meetings, staffing, workshops, and prevention activities held in the agency, as well as the community, directly related to clinical needs and/or client needs. Ensure follow-up of all clients who do not keep appointments. Maintain productivity standards as assigned. Participate in Quality Performance Improvement within the agency (Assignment to QM process workgroups, gathering and analyzing data, attending QM meeting, including but not limited to mandatory all-staff meetings). Qualifications: Masters degree in a mental health-related field. Florida Licensure as a counselor, social worker, or therapist required. Possess good interpersonal and communication skills to deal with public. Must be able to comprehend, speak, and write the English language. Ability to operate general office equipment, especially a computer. Must possess a valid Class 'E' Drivers License with a satisfactory driving record.
Pay Grade: 120
Starting Salary: $39,750.51
