SJC Community Needs Assessment Survey: Your Voice Shapes Our Future

Survey runs from Dec. 1, 2025, through Jan. 9, 2026

St. Johns County Health and Human Services is inviting residents to participate in a new Community Needs Assessment Survey, open Dec. 1, 2025, through Jan. 9, 2026. The survey is designed to help the County better understand the strengths, challenges, and lived experiences that shape health and well-being across our community.

The County is asking all adults who live, work, and learn in St. Johns County to share their perspectives. Responses will help identify local health and human services priorities, gaps in resources, and opportunities to support a higher quality of life for everyone.

“This survey is an essential tool in ensuring every voice in St. Johns County is heard and valued,” said Shawna Novak, St. Johns County Health and Human Services Director. “Understanding community experiences allows us to support programs and services that promote health and wellness for all.”

The feedback collected will guide future initiatives, including programs that support mental and behavioral health, access to care, housing stability, substance-use prevention and recovery, and resources for the elderly. Community input will also help the County identify and remove barriers that prevent residents from living healthy, fulfilling lives.

Survey Details

  • Who can participate: Individuals ages 18+ who live, work, or study in St. Johns County
  • Response type: Voluntary, confidential, and reported only in summary form
  • Personal information: No identifying information of any kind is collected

The survey has been reviewed and approved by the St. Johns County Health and Human Services Department to ensure it meets ethical and privacy standards.
